What is the value of an early diagnosis of dementia in the absence of effective treatment? There has been a lively scholarly debate over this question, but until now patients have not played a large role in it. Our study supplements biomedical research into innovative diagnostics with an exlporation of its meanings and values according to patients. Based on seven focusgroups with patients and their care-givers, we conclude that stakeholders evaluate early diagnostics with respect to whether and how they expect (...) it to empower their capacity to care. They value it, for instance, with respect to whether it explains experienced complaints, allows to start a process of psychological acceptance and social adaptation to the expected degeneration, contributes to dealing with anxieties, informs adequately about when to start preparing for the end of life, informs the planning of a request for euthanasia, or allows society to deal with a growing amount of dementia patients. Our study suggests that information about disease is considered ‘harmful’ or ‘premature’ when recipients feel unable to act on that information in their care. The results of this research offers input to further ethical research. It invites to adopt a care perspective in evaluation and to seek ways to prevent the ‘harm’ that such diagnostic methods can bring about. (shrink)

In accord with common usage we understand that the term "ecology" (from the Greek oikos, "home") refers to our being at home on the earth, and "ecological crisis" refers to the destruction that has been worked to make the earth/home less inhabitable for a variety of species. "Creation" refers to the whole created world and all living beings as gifts from God the creator. For example, in 1990 John Paul II articulated a number of avenues from within the Catholic tradition that relate to the ecological crisis and the Church's corporate responsibility to face it theologically, spiritually, and practically.4 In 1991 the U.S. bishops issued a more focused document, Renewing the Earth, which was drawn largely from the Church's social teaching tradition.5 The purpose of this article is to review some recent Roman Catholic theological literature in English (originally or recently translated) on environmental theology and ethics that can be understood as deep ecology from within mainstream Catholic thought. The contention of these authors is that the ecological crisis is a matter of theology, ethics, and values. IntroductionCerebral visual impairment is an important cause of visual impairment in western countries. Perinatal hypoxic-ischemic damage is the most frequent cause of CVI but CVI can also be the result of a genetic disorder. The majority of children with CVI have cerebral palsy and/or developmental delay. Early diagnosis is crucial; however, there is a need for consensus on evidence based diagnostic tools and referral criteria. The aim of this study is to develop guidelines for diagnosis and referral in CVI according (...) to the grade method.Patients and MethodsWe developed the guidelines according to the GRADE method 5 searches on CVI were performed in the databases Medline, Embase, and Psychinfo, each with a distinct topic.ResultsBased on evidence articles were selected on five topics: 1. Genetic assessment 5.ConclusionIn medical history taking, prematurity low birth weight and APGAR Scores are important. Different questionnaires are advised for children under the age of 3 years, older children and for specific risk groups. In ophthalmological examination, eye movements, specially saccades, accommodation, crowding, contrast sensitivity and visual fields should be evaluated. OCT can show objective signs of trans-synaptic degeneration and abnormalities in fixation and saccades can be measured with eye tracking. Screening of visual perceptive functioning is recommended and can be directive for further assessment. MRI findings in CVI in Cerebral Palsy can be structured in five groups: Brain maldevelopment, white and gray matter lesions, postnatal lesions and a normal MRI. In children with CVI and periventricular leukomalacia, brain lesion severity correlates with visual function impairment. A differentiation can be made between cortical and subcortical damage and related visual function impairment. Some patients with schizophrenia are not told their diagnosis. The moral, clinical and practical issues involved in telling or not telling the patient are discussed. In some cases a relative is told the diagnosis but not the patient. The implications for the family and clinical outcome are outlined. A case history illustrating some of these issues is presented.

Advances in genetics may soon make possible the development of ethnic bioweapons that target specific ethnic or racial groups based upon genetic markers. While occasional published reports of such research generate public outrage, little has been written about the ethical distinction (if any) between the development of such weapons and ethnically neutral bioweapons. The purpose of this paper is to launch a debate on the subject of ethnic bioweapons before they become a scientific reality.

This article deals with the burning issue of moral decision-making by major church assemblies,such as regional and general synods. Moral decisions by church assemblies have createdmany conflicts in churches in the past and at times did an injustice to the prophetic testimonyof churches in society. The question arises as follows: To what extent should church assembliesbe involved in moral decision-making? The central theoretical argument of this study is thatalthough the notion of a ‘biblical ethic’ is valid, synods and council of (...) churches should beextremely cautious and even hesitant to formulate moral decisions because of differences inhermeneutical approaches and the principle that the church is primarily the ‘local congregationof believers’. The church is not in the first instance a national, general or international socialstructure that should pass conclusive resolutions and that testifies by way of moderators orelected church leaders. To unfurl this central theoretical argument, the researcher refers to thecurrent hermeneutical discourses and proposes certain ideas regarding the possible role ofthe church with respect to moral decision-making. The first few years of driving is a critical period when driving skills develop and the driving style is established. While the actual driving skills improve during the first few years of driving, a novice driver’s view of himself/herself as a safe and/or skilful driver also develops rapidly. The aim of this study was to investigate self-evaluated driver safety and perceptual-motor skills among different age groups of young drivers, along with the relationships between self-evaluated skills and driving behaviour. The sample (...) consisted of a stratified random sample of 18–25-year-old drivers from the Finnish driving licence register. The questionnaires, which included the Driver Skill Inventory, Driver Behaviour Questionnaire and background information, were completed and returned by a total of 1,058 participants. While female drivers assessed their safety skills to be higher than their perceptual-motor skills, the opposite was true for males. In both sexes, perceptual-motor skills increased, and safety skills decreased with experience. Perceptual-motor skills correlated negatively with safety skills, lapses and errors, but positively with aggressive and ordinary violations. Safety skills correlated negatively with all DBQ variables. Safety orientation seems to be most clearly reflected in deliberate aberrant driving behaviours. Sex differences were observed in the development of behaviours and skills, perceptual-motor skills only increased with age among males, while safety skills decreased through experience among both men and women. Results showed that driving experience was strongly related to both driving style and the drivers’ view of their skills, highlighting the importance of the first few years of driving. (shrink)

Paul-Mikhail Podosky introduces the notion of other-oriented hermeneutical injustice and argues that non-human animals are often the subjects of such injustice. In this paper, I argue that although the notion of other-oriented hermeneutical injustice is coherent, Podosky’s examples – including his primary case of non-human animals – are not instances of it. I attempt to show that an epistemology of ignorance serves as a better theoretical basis for Podosky’s argument. In the final section of the paper, I discuss a case (...) that, at first glance, seems to be one of other-oriented hermeneutical injustice. However, I argue that ultimately an epistemology of ignorance gives us a better account of the case. (shrink)

: This contribution offers a detailed presentation of an anonymous book on the soul ascribed to the fourteenth-century Franciscan philosopher and theologian Nicholas Bonet. The work is conserved in two manuscripts of the National Library of the Czech Republic in Prague. In both manuscripts the work is almost certainly incomplete. It has a strong focus on the vegetative and sensitive operations of the human soul and on phenomena such as light and colour. There have been two main lines in the literature on restricted games: the first line was started by Myerson (1977) that studied graph-restricted games an the second one was initiated by Faigle (1989). The present paper provides a unified way to look on the literature and establishes connections between the two different lines on restricted games. The strength and advantages of this unified approach becomes clear in the study of the inheritance of the convexity from the game to the restricted (...) game where an interesting result by Nouweland and Borm (1991) on the convexity of graph-restricted games is turned into a direct consequence of the corresponding result by Faigle (1989), by means of this relation. This article puts forth 2 competing notions of the American Dream, 1 radical and 1 conservative (both put forth by Thomas Jefferson), as the basis for 2 competing public philosophies of American democracy and education. This article traces out the ecology of inequality that has determined the context of these 2 competing public philosophies, especially in relation to the evolution of U.S. education. The ideology of the American Dream is still a potent philosophical means for constructing reformist discourses for American (...) politics and education. The rhetoric of the American Dream focuses on the unrealized promises found within the Declaration of Independence and it articulates the need of equality and freedom for all members of our society.
